Waiver

Waiver Forms must be completed for all participants of any activities taking place at Aspire Climbing & Fitness. 

INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later

IMPORTANT!
1) Waivers for minors ages 17 years and under, must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
3) Waivers are valid for two years from the date of signing. If unsure, complete a new one to update your account.
